Submitting a Revised or New Certificate or Degree for Review

Q. What do I submit to the Curriculum Office for a revised or new certificate or degree?
A. Turn in the following items:

  1. Complete the necessary forms, and obtain the appropriate signatures (see what forms to complete)

  2. Make 10 copies of ALL forms, and submit them along with the orignal to the Curriculum Office.

  3. Send any supporting documentation created as an e-copy to curriculum@fhda.edu.

  4. Once you have completed the above-mentioned tasks, your certificate or degree will be placed on the curriculum committee's agenda for future review. You will be notified by the Curriculum Committee Co-Chair if there is a problem concerning your certificate or degree during the curriculum review.
NOTE: To make the catalog deadline date and be included in the appropriate catalog year you must submit all paperwork by the deadline date. If you missed the catalog deadline date, your certificate or degree will go into the following year's catalog.
