If you are creating a new certificate or degree you must complete the:
(1). Request for a New Certificae or Degree form, and
(2). Create a Program Specification Sheet for the certificate or degree, click here for

If you are revising a certificate or degree you must complete the:
(1). Request to Revise a Certificate or Degree form, and
(2). Print out a hard copy of the
Program Specification Sheet for the certificate or degree and manually indicate the changes with a pen. It is more convenient and less time consuming to do this process manually than electronically.
You must complete one (1) form for each certificate or degree; for example, if you are creating/revising a "Web Design" certificate of achievement and an A.A. degree, you must complete one form for each request. On the form, you must clearly indicate what type of certificate or degree it is: skills certificate, certificate of achievement, certificate of achievement - advanced, or A.A. or A.S. degree.
