All courses (new and revised) submitted for review must have a curriculum proposal cover sheet form. Please complete all applicable fields on the form, particularly these fields: course department and number, title, unit, hour, effective quarter (this is the catalog year), and if this is for a 5-year revision or technical changes. In addition, you must have the appropriate signatures on the form. Please contact your division curriculum representative if you have questions or need assistance with the form.

The Curriculum Proposal Cover Sheet form is available in a writable pdf format, and can be completed by: (1) Hand, by printing out the form, or (2). Electronically, using Adobe Acrobat Reader (5.0 or greater), other pdf viewing applications may not work. If you choose to complete the form using Adobe Acrobat Reader, please note that you CANNOT save the form so be sure to print out a copy of the form before closing it up. No matter what methods you choose to complete the cover sheet form, the signature portions of the form must be done manually.
