If your course is due for a 5-year review and it currently holds a distance education mode of delivery, you must complete the Request for Approval of Distance Education Delivery form when you submit your course for review if you want to keep the distance education mode of delivery. You must must obtain signature from the dean of distance and extended learning on the form, in addition to other signatures. Please contact the distance learning office if you have questions or need assistance with the form. If you are creating a new course and would like distance education mode of delivery, you must also complete this form.
