If your course is a hybrid, you must complete the Request for Approval for Distance Education or Hybrid Delivery form.
Hybrid Scheduling Options for Fall 2009

If your course is due for a 5-year review and it currently holds a distance education mode of delivery, you must complete the Request for Approval of Distance Education or Hybrid Delivery form when you submit your course for review if you want to keep the distance education mode of delivery.

You must obtain the signature from the coordinator of distance and extended learning on the form, in addition to other signatures. Please contact the distance learning office if you have questions or need assistance with the form. If you are creating a new course and would like distance education or hybrid mode of delivery, you must also complete this form.
