What Forms Do I Need to Complete?

Listed is a summary of the types of forms you would need to complete if you are doing a 5-year course revision, creating a new course, revising a certificate/degree, or creating a new certificate/degree. A new form must be completed even if there are no changes because there may be new information that is needed. All forms must have the appropriate signatures. If you have questions or need assistance completing any of the forms please contact your division curriculum representative.


REQUEST FORMS TO COMPLETE
5-Year Course Revision 1. Curriculum Proposal Cover Sheet (required)
2. Content Review Matrix (if there is a prerequisite/advisory)
3. Distance Learning Form (if requesting distance learning)

New Course 1. Curriculum Proposal Cover Sheet (required)
2. New Course Request Form (required)
3. Content Review Matrix (if there is a prerequisite/advisory)
4. Distance Learning Form (if requesting ore retaining distance learning or hybrid delivery mode)

Revised Certificate/Degree 1. Request to Revise a Certificate or Degree Form (required)
2. Update the Specification Sheet for the certificate or degree (required)

New Certificate/Degree 1. Request for a New Certificate or Degree Form (required)
2. Create a Specification Sheet for the certificate or degree, click here for specification sheet example. (required)

Course Number or Title Change No form needed, submit a memo with the division dean's signature.
*You must revise any certificates/degrees impacted by this change.

Delete a Course(s) No form needed, submit a memo with the division dean's signature.
*You must revise any certificates/degrees impacted by this change.

Prerequisite/Advisory Statement Change Only No form needed if it is a minor wording change to the prerequisite/advisory statement, submit a memo with the division dean's signature.

Distance Learning and/or Hybrid Request (ONLY)

2. Submit original with signatures and one additional copy.
