Which Advisory and Prerequisites Forms to Use?
Per Title 5, if advisories and/or prerequisites are requested for a course (referred to as the target course) the appropriate content review scrutiny level forms must be completed and signed off by the Matriculation Coordinator, Cheryl Woodward, (x8780, office: SCS 220) PRIOR to submission to Mary Clark. Make sure to include the course outline when submitting the advisory/prerequisite forms to the Matriculation Coordinator.

Level 1 Scrutiny Forms Must Be Completed For:
ALL advisories for the target course and ALL prerequisites WITHIN the discipline of the target course.

1. Communication Advisories (Example)
   * EWRT 1A complete Content Review Matrix A (Pdf version, Word)
   * EWRT 211/READ 211 (LART 211) [formerly EWRT 100/READ 100 (LART 100)] complete Content Review Matrix B (Pdf, Word)
   * EWRT 200/READ 200 (LART 200) complete Content Review Matrix C (Pdf, Word)
   
   
2. Math Advisories (Example)
   * Math 114 or equivalent complete Content Review Matrix D (Pdf version, Word)
   *Math 212 (formerly MATH 112) or equivalent complete Content Review Matrix E (Pdf, Word)
   * Math 210 or equivalent complete Content Review Matrix F (Pdf, Word)
   
   
3. Other advisories and prerequisites WITHIN the discipline of the target course (Example)
   * complete a Content Review Matrix G for EVERY advisory and/or prerequisite requested (Pdf, Word)
* complete a Content Review Matrix G for special projects course - the minimum that is required on the form (Word)

Level 2 Scrutiny Must Be Completed For:

1. Prerequisites OUTSIDE the discipline for transferable courses. Complete the two bulleted items below and submit to Cheryl Woodward, Matriculation Coordinator/Counselor, (x8780, office: SCS 220) PRIOR to submission to Mary Clark.
   * Find 3 CSU/UC's that have and equivalent course to the transfer/target course, photocopy or print out the catalogue descriptions of the courses and their prerequisites. (For assistance in finding the 3 CSU/UC schools, contact Renee Augenstein at ext. 8631 or augensteinrenee@fhda.edu.)
   * Complete a Content Review Matrix O (Pdf version, Word) for each prerequisite for the transfer/target course.
   
   
2. Limitations on Enrollment complete the appropriate section on Form H (Pdf, Word) which should be used for any of the situations below.
   * Prerequisites for cohorts, such as Honors or performing groups
   * Prerequisites based on Government/Licensing/Certification Regulations
   * Prerequisites based on Health and Safety
   Or for (Example)
   * Prerequisites for entrance into a program, such as Nursing, pick one of the courses and complete Content Review Matrix G (Pdf, Word)
